The Patient Journey

Hospital-Level Care,
Without the Hospital.

From the moment a physician refers a patient to the moment they complete their acute episode at home, BeHome Hospital manages every clinical detail — 24/7.

The Process

From Referral to Home-Based Acute Care

Referral01

Physician or ER Referral

A referring physician, emergency department, or care manager contacts BeHome Hospital. We accept referrals by phone, secure fax, or our partner portal — 24 hours a day, 7 days a week.

Typical response time: under 2 hours
Assessment02

Clinical Eligibility Review

Our board-certified medical team reviews the patient's clinical status, diagnosis, home environment, and support system to confirm eligibility for hospital-at-home care.

Completed before patient discharge
Onboarding03

Home Setup & Admission

Our clinical team visits the patient's home to install monitoring equipment, establish IV access if needed, conduct a full admission assessment, and brief the patient and family.

Same-day or next-day admission
Active Care04

24/7 Acute Care at Home

Patients receive hospital-equivalent care: daily physician visits, nursing assessments, IV medications, lab draws, imaging coordination, and continuous remote monitoring with real-time alerts.

Average length of stay: 4–7 days
Transition05

Discharge & Follow-Up

Upon clinical resolution, our team coordinates a structured discharge with the referring provider, schedules follow-up appointments, and ensures a warm handoff to outpatient care.
